Meet Diana Ramirez

We are proud to welcome Diana Ramirez as an integral member of our elder care support team. Diana is our Elder and Disability Care Coordinator. She has a Master Degree in Gerontology and a Bachelor Degree in Social Work. Diana is a Certified Care Manager (CMC), a designation awarded by The National Academy of Certified Care Managers. She is a member of the National Association of Professional Geriatric Care Managers and the Life Care Planning Law Firms Association (LCPLFA). She serves on LCPLFA’s Ethics Committee. Diana has over 16 years of experience in working with seniors and the disabled in various health care settings.

What is a Geriatric Care Manager (GCM)?

A GCM is a health and human services specialist who acts as a guide and advocate for families who are caring for a frail elder or a disabled adult. A GCM assists clients in attaining their maximum functional potential. Geriatric Care Managers are able to address a broad range of issues related to the well-being of the client. They also have extensive knowledge about the costs, quality, and availability of resources in the communities.

What are the services provided by a Geriatric Care Manager?

  • Helping families evaluate and select appropriate level of housing or residential options such as Independent Living Facilities, Assisted Living Facilities, Residential Care Facilities or Skilled Nursing Facilities
  • Selecting Home Care services which allow the client to remain in the home safely and securely.
  • Managing Medical Care by attending doctor appointments and facilitating communication between the health care professionals, client and families.
  • Providing the opportunities for the client to engage in social, recreational or cultural activities to enrich the quality of life.
  • Providing safety and security by monitoring the client in their home with the management of medication, recommending technologies to assist in the safety of the client and observing any changes that may result in the declining health of the client.

What services are available from Diana Ramirez?

Assessment.  An assessment evaluates the overall functioning of the client. The GCM will assess the client’s current nutritional and mental status and evaluate the current medication regimen and the safety of his or her current home environment. An analysis of care options and the cost of each option are included. A full written report includes recommendations and formulates a viable plan of care for the client as an individual.

Monthly Maintenance.  Diana can also be retained for a monthly management fee of $250 to assist with the weekly management of medications, attendance of one medical appointment, adherence to a plan of care and weekly communications with family members and health care professionals.

Additional Services.  An unlimited amount of monthly services can also be contracted for at an hourly rate of $75 per hour.
